Incentive Guidelines
Payson City Employee Wellness Program is available to employees, spouses and children. Unless otherwise stated, only employees are eligible for the incentive reward.
Incentives must be completed at least 80% to qualify for the incentive award. Logs/calendars must be turned in to Sarah to receive the reward.
Participants must sign-up for each incentive; you just don’t get automatically rolled over to the next incentive.
